Introduction

Thank you for taking part in this survey and for providing valuable feedback on the introduction of our new digital consent form for your son/daughter's immunisation undertaken in school. You may be aware from recent media coverage that the NHS is seeking to transform much of its paper-based communication with patients, with an aim of going paperless by 2020. The feedback you provide is vital to help us understand your experience of this new e-Consent form, and to help us improve the service for future immunisation programmes.
