What happens when you come out of hospital after an overnight stay?

The City of London Corporation is currently looking at people’s experiences of when they are discharged from hospital (sent home after staying in hospital overnight for assessment or treatment).  We know that our residents visit a variety of hospitals and want the support, advice and information to be available wherever they are treated, is high quality and is given at the right time and in the right place for the person.

We are interested in hearing from people (or from Carers of people) who live in the City of London and have experienced an overnight stay in hospital.  This survey should not take more than 10 minutes for you to complete.

If you have not stayed overnight but want to share any comments or ideas please complete your details (Q1 to Q6) and Q18 to Q20 only.
