This survey is from Young Healthwatch Northamptonshire and is being done on behalf of Healthy Young Daventry. Healthy Young Daventry is a project that brings together local services to promote the health and wellbeing of young people living in Daventry town. One focus for Healthy Young Daventry is building emotional resilience and helping reduce rates of distress, including self-harm. We want to make sure that we hear your voice so that we can see what is working well and what could be improved. 

This survey is for parents of young people between the ages of 11 and 19 living in Daventry. It asks about your young person’s physical health, lifestyle, emotional health and how they manage difficult emotions, to see if there is anything that services could provide to help. It also gives you – as parents - the chance to let us know what you feel would work for your family and how you would like to receive services.

At the end of the survey there will be some contact information for support if you feel that you or someone you know is or may be affected by the subjects asked about in the survey.

We do not ask for your name and no one will know who said what. We do ask for information about you as we would like to know how diverse the people who responded to the survey are. We keep all this information secure and the data will be shared with the people who run and make decisions on services for young people in Daventry. Healthwatch Northamptonshire will publish the report in 2020.
