This survey is from Young Healthwatch Northamptonshire and is being done on behalf of Healthy Young Daventry. Healthy Young Daventry is a project that brings together local services to promote the health and wellbeing of young people living in Daventry town. One focus for Healthy Young Daventry is building emotional resilience and helping reduce rates of distress, including self-harm. We want to make sure that we hear your voice so that we can see what is working well and what could be improved.

The survey is for young people between the ages of 11 and 19 living in Daventry. It asks about your physical health, lifestyle and emotional health and includes questions on how you manage difficult emotions and questions on anything that services could provide to help you with this. It also gives you the chance to let us know what things you feel would work for you and how you would like to receive health and wellbeing services.

At the end of the survey, there is some contact information for support if you feel that you or someone you know is or may be affected by the subjects asked in the survey.

We do not ask for your name and no one will know who said what. We do ask for information about you as we would like to know how diverse the people who responded to the survey are. We keep all this information secure and the data will be shared with the people who run and make decisions on services for young people in Daventry. Healthwatch Northamptonshire will publish the report later in the year.

You can find out more about this and data security by asking us or going to www.healthwatchnorthamptonshire.co.uk/privacy

If you would like more information about the survey or Healthy Young Daventry project or would like to tell us more, then please contact us. You will find the details again at the end of the survey. As part of this work, we will be holding workshops to help develop services that work for you and young people in Daventry Town, if you would like to be involved then please contact us.
