Path to Excellence phase two patient survey - Medicine and emergency care survey (East and North Durham)

We are reviewing how health services across South Tyneside and Sunderland can be improved and would like to understand what is important to you.

This is a survey about your experiences of emergency care services at Sunderland Royal Hospital following a visit to the hospital’s Emergency Department (A&E), or following an emergency admission to the hospital for emergency treatment or surgery.

Who should complete the survey?

The survey should be completed by patients who have attended the Emergency Department (A&E) at Sunderland Royal Hospital, and may have received emergency care, or been admitted for emergency treatment or emergency surgery at Sunderland Royal Hospital in the last two years.

We are particularly seeking people who live in either the East Durham or North Durham areas to fill in the survey, but you must have accessed emergency care at Sunderland Royal Hospital in the past two years.

The deadline for completing the survey is midnight on Wednesday 12 February, 2020.
