Thank you very much for taking part in this survey. We would like to find out how Covid-19 has impacted the support you and your family receive from the Woodfield Child Development Service (WCDS). We are really keen to find out your views about how you would like services to look like. We are particularly interested in how you have found the shift towards using telephone or video for clinical appointments as a result of Covid-19. This will really help us to plan services going forward and to make sure that our offer is flexible enough to meet diverse family needs.

This survey should take about 10 minutes to complete. The overall findings will be fed back to the Child Development Service and may be used for publications, however, all the responses will be kept anonymous. Your participation in this survey is completely voluntary, and you can choose to withdraw your particiption at any time, without giving a reason.
