For carers, families, or friends of people who have been personally affected by suicide.

Heads On is the charity for Sussex Partnership NHS Foundation Trust (SPFT), delivering projects to enhance NHS mental health care. Our approach is simple - to do everything we can to help make things better for the people who come to SPFT for help.

We are currently working with SPFT's Suicide Prevention team, on a range of projects that will support SPFT's Towards Zero Suicide strategy; both raising awareness of mental health and suicide prevention in our local communities and developing projects that make a real contribution to reducing suicide in South East England.

Your response to this survey will be used to develop new initiatives that will help prevent suicide in Sussex. You can complete the survey anonymously.

We know that talking about suicide can be difficult, if at any point you are finding it hard to complete this survey please stop.
