Health Improvement Survey 2018/19

The Health Improvement Team greatly appreciates your time in completing our survey. We work hard to provide the best service we can but we rely on your feedback to improve what we do every year. This survey is to be completed by both the PSHE Coordinator and Headteacher of every school. All questions with an asterisk require an answer, all other questions are optional. 

 

This survey should take a maximum of 5 minutes to complete.  Every tenth survey submitted will win a Boots voucher. The winner will be announced in the autumn issue of our termly newsletter 'Stepping Up To Health'. This survey closes at midnight on 12 July 2019.
