Transformation Intervention Pathways - Expression of Interest

About this survey

Thank you for taking the time to get involved in the All Age Transformation co-design programme to improve the mental health and learning disabilities services that Leicestershire Partnership NHS Trust provides.

We're in stage 3 of the co-design transformation journey and are creating the detailed service designs. This survey is for those interested in getting involved in the development of 'Intervention Pathways' (also known as treatment or care pathways). You can choose which pathway/s you are interesting in developing from the list below.

It is expected that each pathway will require a commitment of around 4 days of workshop (spread across several weeks) plus time to review evidence.
