1. SURVEY INFORMATION

 Hello,

This project aims to gather many different people’s views and experiences (patients, carers, the general public and a range of healthcare professionals etc.) of psychological support within cancer and / or palliative care services. Before deciding if you would be happy to take part in this project, please take your time to read this information sheet, which explains what your participation would involve. 

Why is this project being undertaken? 
· To understand what emotional / psychological support people need when accessing cancer or palliative care services.
· To understand people’s experiences of receiving / not receiving emotional or psychological support in cancer and palliative care services across Birmingham and Solihull.
·To gain people’s ideas as to how psychological support within cancer/palliative care services could be improved.
·This project will produce recommendations to develop the emotional / psychological support provided by services across Birmingham and Solihull.

What will happen if I agree to take part in this project? 
This online survey will take approximately 10 minutes to complete.

The survey will ask questions:
· About your age, location, ethnicity, etc.
· About your experience of psychological care/support in cancer and/or palliative care services.
· About your emotional / psychological experiences (e.g. stress, worry, low mood etc.) when accessing cancer and/or palliative care services.

There are no right or wrong ‘answers’ to these questions: we just want to hear what you’ve got to say. 

It’s important you know that what you tell us will not change or affect the care you are currently receiving or may receive.

What are the possible disadvantages / risks of taking part?
Whilst you complete this survey, you will not be at any greater risk of discomfort than you would in your day-to-day activities, however some questions are sensitive in nature – you are free to leave any questions blank if you wish. At the end of the survey you will be provided with information for telephone support services (Macmillan Cancer Support Line: 0808 808 0000, Marie Curie Support Line: 0800 090 2309) that you can access if you've been affected by the content of this survey. 

What are the possible benefits of taking part?
Your participation will provide useful information to enable this project to understand what the views / experiences are of people who have engaged in cancer and/or palliative care services across Birmingham and Solihull.

What will happen to my survey answers?
All of the information collected from online surveys will be kept confidential and will only be used for this project.

This project will produce reports/presentations of the work it has completed; some of these reports/presentations may include anonymous statements from your survey, to help illustrate what you have to say. 

Do I have to take part?
Your participation is completely voluntary; we only want you to take part if you feel comfortable to do so. When completing the survey you can stop at any point without your answers being saved. However, once you have submitted your responses, you won’t be able to withdraw your survey.
