Improving Warfarin Services

NHS Wandsworth Clinical Commissioning Group wants to improve anti-coagulation services in Wandsworth for all warfarin patients and their carers, no matter whether they go to hospital or their GP practice to receive treatment.

We want to make the service easier to use and make it a better experience for patients and carers who use it, and we want to improve the overall quality of the service too.  If you currently use anti-coagulation services in Wandsworth, either as a patient or carer, we would welcome your views on the service you are receiving and the improvements you would like to see.

The feedback you provide in this form will ONLY be used to help improve services.  You do not need to give your name or any information relating to your health.
