At Carer Support Wiltshire we are keen to understand the impact on unpaid carers of revised ways of working and more virtual approaches with GP Practices.

We are reviewing carers’ experiences during the Covid 19 pandemic. We would like to hear from you about your particular experience of your surgery during the period April 2020 to February 2022.

We have created a short questionnaire and it would be helpful to us if you could take the time to complete this. It should take about 10 minutes to complete. The survey results will be fed back to NHS England and Improvement to learn from unpaid carers experiences during COVID.

All the information you provide is completely anonymous.
