Facility Information, Policies and Procedures

This is a pre and post Infection Prevention and Control assessment form designed to be used at Healthcare Facilities to inform training and teaching programmes. The assessment looks at the following areas:
 - IPC Infrastructure
 - IPC Staffing and Training
 - IPC Policies
 - IPC Training
 - Occupational Health
 - COVID-19 Management
 - Waste Management
 - Cleaning and Sterilisation of Medical Devices
 
The post assessment will be used to measure improves made and residual knowledge transfer.

To be completed by hospital manager and returned to IPC focal person before baseline assessment date  OR to be completed during an interview with either the healthcare facility manager, or head of clinical or head of nursing.

Definitions:         
Nurses: nurse assistants, midwives and student nurses
Doctors: doctors, dentists and medical students
Ancillary staff: SSD operators, laboratory, dental, and medical technicians, cleaners, GA’s, porters
Allied health staff: physiotherapists, OT’s, dieticians etc.
Community caregivers: people attending to patient needs and care including family members (not just visitors)

T
