About this survey...

Thank you for taking the time to fill out this survey. It should take about 2 - 3 minutes.

There is a quite a lot of space between questions, so the survey looks longer than it actually takes!
We value your feedback, especially any extra information that you include in the comments boxes. This gives us the detail that we need to make the changes that you suggest.

This information is all confidential and anonymous. We are obliged to ask certain questions to comply with NHS rules for how GP Surgeries are governed.

This survey is to help you let us know about the things we can do to help improve YOUR experience.

How you can help us
There are some simple things that you can do - such as:
 - Provide a personal email and mobile contact - so we can reduce our postage costs and text you appointment reminders
- ALWAYS responding to our FIRST invitation to come in for an appointment or vaccination or
- ALWAYS letting us know as soon as you can if you cannot get to an appointment
- Sign up for our online services and book appointments, request repeat medication and change details online
- Only use urgent on the day appointments - when really unwell and it is actually urgent
- Self care when you can
- Order your repeat medicine on time and not too early or too late

Things that we are trying to do at the moment:
- Investing in more appointments - we have increased the number of GP appointments available and when fully staffed we are often able to offer a non-urgent GP appointment within 1 week.
- We have a plan for 3 TV Notice screens - 1 for waiting room - 1 for the foyer and 1 for the lower corridor.  We are making slow progress on this but we now have cabling in place.  We are awaiting NHS IT Teams to help us with a new computer.
- Due to some incidents of challenging behaviour and rudeness towards our team we will be installing CCTV - we are awaiting NHS England to help us as the system will need to link to NHS services for security.
- We would like to use email more for our letters of invitation - this will help us reduce the cost of postage. Please can  you provide us with an up to date personal email address.

About our building
This year we bid for money from the NHS for a new building. However, even though our plan was second on the list for this area and the CCG and NHS England acknowledge a new building is much needed and our local NHS CCG supported us funds were not made available.
