Are you a Carer?
A Carer does not mean you have to receive carers allowance but that you routinely look after a sick child, elderly or disabled person. You can be a family member, friend, neighbour or a paid helper.
If you would like this to be recoded on your records that you are a Carer please leave your name and Date of Birth.
Please note if you are a carer aged under 65 please call the surgery and book in for your free flu vaccination.
