Please include information about why the fundraiser chose the BDA to fundraise for, the inspiration behind the fundraising, what they did to fundraise and how much was raised during the fundraising activity (the award will not be based on how much was raised but on how creative the fundraising was), also do let us know when the fundraising took place.

If you would like to send in any pictures or clippings of the fundraising / fundraiser - please send a email to fundraising@bdadyslexia.org.uk with the subject as Fundraiser Award and the nominee name.

A distinguished panel of judges from the sponsoring organisation will select the winner. Winners will be notified and invited to attend the dinner where they will be presented with their award. By submitting this form, you give the BDA permission to use the information contained within it as part of your awards nomination.

All nominations must be received by Friday 8th April 2022.
