Relocating the RNHRD's Bath Centre for Fatigue Services- have your say

This survey relates to the Royal National Hospital for Rheumatic Diseases (RNHRD) Bath Centre for Fatigue Services (BCFS) - you do not need to have been a patient of this service to take part in this questionnaire. The survey will take around five to ten minutes to complete.

The  BCFS provides services for people experiencing longstanding fatigue that is having a significant impact on daily functioning and activities. This includes those with Chronic Fatigue Syndrome/Myalgic Encephalomyelitis (CFS/ME) and adults experiencing post-cancer related fatigue.

We're proposing to relocate the services currently provided by the BCFS from their current location at the Mineral Water Hospital site in Bath to a purpose built RNHRD and Therapies Centre on the Royal United Hospitals Bath NHS Foundation Trust (RUH) Combe Park site in 2018, and we’d like to hear your views. The new Centre will bring together a number of outpatient services from the RUH and RNHRD which support patients to live independently in the community, you can find out more here  In the new RNHRD and Therapies Centre, patients will continue to be seen and treated by the same team, providing the same high standards of care. Only the location will change,

This proposed relocation only affects the services provided from the Mineral Hospital site - satellite clinics will continue in their current location.

This is part of a careful and phased approach to relocating all RNHRD services provided from the Mineral Hospital site to the RUH or appropriate community based settings.

We welcome your views about how you would like to see the BCFS provided now and in the future. Your feedback will help us in our planning to ensure we continue to provide the best service for our patients.

Please be assured the information provided by you in this questionnaire will not be used in a way which would allow identification of you as an individual.
