Please complete this form if you would like to apply for a place on the UK SMART Recovery Family & Friends training course.
Please note that if you have not already done so you will be required to complete the Getting SMART and Facilitator course before being enrolled onto the Family & Friends module.
 
Before continuing please ensure you have read and understood the eligibility criteria below.
 
Eligibility
The UK SMART Recovery Family & Friends training is currently available to individuals who meet one of the following conditions: -
* You are a person in recovery (stable for at least two years), have lived experience of a friend or family member with addictive behaviours and wish to facilitate or co-facilitate a stand-alone meeting in your local community (not connected to a service/treatment provider) .
 
* You are not personally in recovery but have lived experience of a family member or friend with addictive behaviours and wish to facilitate or co-facilitate a stand-alone meeting in your local community (not connected to a service/treatment provider) .
 
* You are a staff member or volunteer who has been nominated as a 'Family & Friends Champion' by one of our partner organisations.
