Speech & Language Therapy Service - Darlington

This survey has been developed by Tees Valley Clinical Commissioning Group to review the Speech & Language Therapy Service that is offered across Darlington, provided by Co Durham & Darlington NHS Foundation Trust.  We would appreciate you taking the time to answer some questions regarding the service and your experiences to ensure that it is meeting the needs of children, young people and families with a speech, language, and communication difficulties.

1.When were you first aware your child had speech, language and/or communication needs?

3.If your child's needs were picked up by a health visitor, did they put any additional support in to help develop your child's speech/communication?
