2020/ 2021 End of Year Survey

Thank you for choosing to complete this survey. The responses are collected anonymously and the raw data will not be shared with any third parties. The responses will be collated into a report which will be shared with relevant parties across Education, Health and Social care, as well as placed upon our website, in order to inform professionals of current issues affecting the lives and wellbeing of parent carers who have a child/young person with Special Educational Needs or Disabilities (SEND) in Peterborough. This survey is for Parent Carers who live in Peterborough only.

As a thank-you for completing this survey we are offering 100 parent carers the opportunity to win a set of tickets/ passes to a local family based activity such as bowling or the theatre. To be in with the chance to win, when you have completed the survey take a screen shot of the thank-you for completion page and email this to office@familyvoice.org along with your contact details. We will then be in contact with the first 100 parent carers whose names are drawn to complete a form to select their preferred choice of family based tickets and passes.
