About the study:

Thank you for your interest in my Questionnaire study, which is intended to improve care for the future. Reading and completing this questionnaire should take no longer than 10 minutes of your time.

The results of this questionnaire will help to compliment the ongoing work of the Embedding study to embed diabetes structured education.

Participation is entirely voluntary and the data you supply is anonymous – you will not be asked your name, contact details or anything else that could identify you personally. You do not have to take part and this will not affect your care in any way.

This is a questionnaire study about ways that patients learn about their health. We are particularly interested in Cardiovascular disease and Type 2 diabetes. There are various methods by which patients may improve their knowledge about and skills to manage their health conditions. We are interested in finding out how people with these conditions want this information to be delivered, for example face to face, leaflets, internet or telephone, and what factors affect their preferences. Understanding this may help healthcare providers ensure that information they provide is given in the most useful ways, and improve the experience of patients.

You have been given this questionnaire because of your health background. The questionnaire takes less than 10 minutes to complete and asks various questions about you, followed by your preferences about how would like to receive information, knowledge and skills in the context of healthcare, and reasons for these preferences.

At present the questionnaire is only available in English, although we hope to translate it into other languages in the future. If you have any trouble understanding the language, an English-speaking friend or family member can help you complete it, if you wish.

The data collected will be analysed to look for trends in how patients like to receive educational information about their health conditions. The data collected as part of this study may be used, in part or in whole, for the writing of educational projects such as a Master's Degree or a PhD. Data will be stored on password protected computers and only accessed by study personnel or by authorized sponsor or University Hospitals of Leicester NHS Trust (UHL) personnel for audit and monitoring purposes. 

The University of Leicester is the sponsor organisation for this study based in the United Kingdom. We will be using information collected from you in order to undertake this study and will act as the data controller for this study. This means that we are responsible for looking after your information and using it properly. The sponsor will keep identifiable information about you for 5 years after the study has finished.

Your rights to access change or move your information are limited, as we need to manage your information in specific ways in order for the research to be reliable and accurate. If you withdraw from the study, we will keep the information about you that we have already obtained. To safeguard your rights, we will not be using personally-identifiable information. 

As this research study involves a questionnaire to capture information about you it is anonymous and therefore University Hospitals of Leicester NHS Trust (UHL) will not have access to your name, NHS number, and contact details to contact you about the research study. Individuals from the sponsor and regulatory organisations may look at the information provided by you in the questionnaire to check the accuracy of the research study. The UHL Trust will pass these details to the Sponsor and regulatory authorities along with the information collected from you. The only people in the sponsor organisation who will have access to information will be people who audit the data collection process.
