Epilepsy and climate change

In this survey, we hope to find out what people affected by epilepsy think about climate change and whether they feel it is having an impact on their health.  Climate change is often referred to in the media and elsewhere as global warming but for the purposes of this survey, we refer to it as climate change throughout.
We would like to begin by asking you some questions about climate change, and then we will ask you some questions about yourself and your epilepsy, or that of someone you care for.
