Vaccinator Application
This is a pre-screening questionnaire to enable you to sign up to be one of our vaccinators. We have tried to make the process as simple as possible.
Our clinics operate under a patient group direction (PGD) so you will need to be a member of a registered profession that can sign and vaccinate under a PGD. A list of these is here.
If you are able to complete this questionnaire then you will automatically be entered into the next stage of the process. If you're unable to answer any of the questions yet, please come back later when you have the necessary information, and try again.
Here is some important key information for everyone:
- Hepatitis B vaccination is not mandatory but we recommend you have it.
- We can offer COVID-19 vaccination if you have not yet had this done. You are a Frontline Health and social care worker. However, we would request that you make a reasonable commitment to taking a regular shift(s) for a minimum of three months. Ideally this would be done before the first shift, but we are in part dependent on how many doses are available for a clinic and whether it is more sensible to wait until the end of the shift to empty a vial. We appreciate your flexibility in this regard.
Doctors: If you are retired, or have cancelled / removed your GMC registration, please re-register here. If you have difficulties doing this, please email us at capccg.mereside-cv19sops@nhs.net as we have been offered help to resolve this.
Being on the performers list for GPs is not required.
Nurses: If you are retired, or have cancelled / removed your NMC registration between 1-Mar-2015 and 30-Nov-2020, please re-register here. If you have difficulties doing this, please email us at capccg.mereside-cv19sops@nhs.net as we have been offered help to resolve this.
Paramedics: If you have de-registered from the HCPC in the last three years, you will have automatically been included on the covid-19 temporary register. You can check this here. If you are not an active paramedic or on this register you will not be able to vaccinate for us.
Pharmacists: If you are retired, or have cancelled / removed your GPhC registration, please re-register here. Some key information is available here.
