EPCF SEND Survey 2021 - Parents and Carers 

Ealing Parent Carer Forum is committed to improving the lives of children, young people and adults with Special Educational Needs and/or Disabilities (SEND) in Ealing. To do this we would like to hear about your experiences as a parent or carer about Ealing's education, health and social care services.

 

Your responses will be collated and anonymised before being shared with services in Ealing. We will never share your personal data with anyone. The only exception to this would be if your comments identified a safeguarding concern which would be brought to the attention of the relevant team(s). 

If you care for more than one child/young person/adult who receives SEND Support with or without an Education, Health and Care Plan (EHCP) please complete a form for each of them.

This survey contains 20 questions (all of which are optional)and should take no longer than 15 minutes to complete and will close by midday on the 14th October 2021
