What is the survey about?
This survey is about getting your opinions on NHS service wait times. Your feedback is very important in helping us understand how you use services at present and how you might do so in the future. The survey shouldn’t take more than 10 minutes to complete. The information will be used to make improvements to research on NHS wait times.

Who should complete the survey?
The questions should be completed by you. If you need help in completing the survey, the answers should be given from your point of view – not the point of view of the person who is helping.

The information you give
Participation in this survey is voluntary. The survey is entirely anonymous so nothing that could personally identify you will be stored. Completion of this survey is taken as your consent, and, because the survey is anonymous, you cannot withdraw your data once submitted.

Completing the questionnaire
The survey consists of multiple choice and open-ended questions. For each question, please select the most suitable option from your point of view. For some questions, an ‘other’ option is available if the choices given are not relevant. If you choose this option, you can give more details in an open text box if you want.
