The purpose of this community research survey is to find out about your current patient experience with local care services and to hear about issues you may have with the service that you have received.

The survey is for people who have received mental health services but are not currently attending St Mary's clinic. There is no upper age limit for the questionnaire.
To ensure confidentiality, please do not write your name on this survey.

* 1. How long have you been receiving support for your long term mental health condition?

* 2. How often do you have GP appointments to discuss your long term mental health condition?

* 3. During your recent GP appointment, did you feel the staff spent enough time with you?

* 4. During your recent GP appointment, did you feel the staff listened carefully to you?

* 5. Have you noticed any rapid weight change as a result of taking your prescribed medication?

* 6. If you experienced a rapid change in weight, did the healthcare staff talk to you about following a healthy lifestyle? (e.g. moderate diet, alcohol intake, smoking).
