As part of our strategic plan which supports people living safe, healthy and independent lives in their own community, we are reviewing the eligibility criteria for accessing North Lanarkshire Health and Social Care's Community Alarm Service (CAS).

The aim is to review the current eligibility criteria for accessing CAS following a consultation with service users, carers, partner agencies and the learning from other models of access (eligibility, referral and assessment) operating elsewhere in Scotland. 

This short survey can be completed from Monday 20th November 2017.  The closing date for all responses is Monday 18th December 2017.
