About the survey and the person you care for

This survey is for people who care for someone who has been in hospital at least overnight and has been discharged since Summer 2020.

During the pandemic procedures for hospital discharge have changed to ensure people are only in hospital if they have a clinical need to be there.

Action for Carers and Healthwatch Surrey want to understand whether hospital discharge processes are working well for carers, or if there are problems that need to be addressed to ensure people get the treatment they need and are discharged safely.

This survey will take 10-20 minutes to complete.  We understand your time is valuable, and everyone who completes the survey can choose to be entered into a prize draw for a £50 voucher.

If you’d like a paper copy of this survey posted to you, or if you would like support to complete the survey please get in touch with us:
- Action for Carers on 0303 040 1234 option 1
- or email CSAdmin@actionforcarers.org.uk
- or send an SMS to 07592 787 533.

A carer is someone of any age who provides unpaid support to family or friends who could not cope without this help. This could be caring for a relative, partner or friend who is ill, frail, disabled or has mental health or substance misuse problems.  The carer does not have to live with the person they are caring for.
If you are a carer and you have been in hospital yourself we would love to hear from you separately.  If you are willing to share your experience with us please contact us by any of the methods listed above.
