* 1. Is there an agreed definition of patient empowerment at National or Regional level where you are? If so, what is it?

* 3. Is there a National or Regional strategy for implementing patient empowerment measures (education, supporting self-management in chronic diseases etc.)? If so, please supply links to allow us to access these.

* 4. If local strategies do exist, can you point us to individuals who are leading these initiatives, so that we could engage with them to get their perspective on how implementation is progressing?
