Mentor Registration Form

Thank you for your interest in becoming a mentor on the CSW Enterprise Mentoring programme!

Mentoring a young person aged 12-15 years of age can have a huge impact. Not just on their learning, motivation and knowledge of the wider world of work, but also on you as a mentor as well.

It's important to know what you're signing up to when you become a mentor. We ask you to make a commitment to:
  • Attend a mentor induction
  • Undergo an Enhanced DBS check (if necessary)
  • Meet with your mentee on a regular basis (at least one hour each month for a minimum of 6 months)
  • Maintain a record of meetings, outcomes and actions
  • Inform the school promptly if you're not able to keep an appointment and make alternative arrangements
  • Communicate with your mentoring coordinator regularly and provide feedback
