Lockdown 2021 SEND Provision Survey - Bexley

This survey is for parent carers with children and young people aged 0-25 with Special Educational Needs and/or Disabilities, living or attending school in the London Borough of Bexley, to find out what services and support are currently available to you and your family and what extra support you need but do not have.  

All information will be kept anonymous.

You can fill this survey out more than once if you care for more than one child with additional needs.  It shouldn't take more than 10 mins to complete.

The deadline for completion is 31st January 2021.

The summary information will be used to let the Local Authority know what is and isn't currently working for families of children and young people with SEND, and may also be shared with other Parent Carer Forums across London to inform the advice given to Central Government and the Department for Education about how best to support our children and young people.
