The staff at Corstorphine Primary and the Parent Council are interested in finding out the attitudes of parents, guardians and carers towards Food Provision.

We are keen to get as many responses as possible and would be very grateful if you could take the time to complete this short questionnaire. There are 19 main questions which should take approximately 10 minutes to complete. There are also comments fields should you wish to add more details.

If your responses to the questions are different for children in different classes then please complete the questionnaire as many times as required, selecting the relevant year group in Question 1 each time.

Please note that all answers will be treated anonymously so neither the school nor the Parent Council will be able to identify who has completed any response.
