Dear Patient,
Due to the current risks associated with the spread of Coronavirus (COVID-19) we need to prevent patients attending the GP surgery to collect prescriptions. You have received this survey to complete because you receive medicines regularly and do not currently have a nominated pharmacy; by having a nominated pharmacy will mean we can send your prescriptions electronically to the pharmacy meaning you do not have to come into the surgery.
You need to ensure you have nominated your regular pharmacy before 5pm on Tuesday 31st March.
GP practices are not allowed to automatically nominate the nearest pharmacy to your registered address on your behalf but can do so with your consent. To do this please complete the questions in this survey and we will update your medical record and will send you a text message the details of the nominated pharmacy.
You can always change the nomination by asking a new pharmacy to nominate you instead. We want to work with you and the community pharmacies serving our population to ensure patients receive their vital medicines on time whilst minimising risk to all by using electronic means to order and process prescriptions.
