Please answer the following questions to record the financial impact suffered by your company which has been caused by COVID-19 and which is currently unresolved and in dispute under your contract with the employer.  

The survey asks for details on up to 5 contracts that have been impacted. If you are unable to provide 5, please provide as many as you can as this is still very valuable information.

Please note, the survey is anonymous and the information will be used to inform actions that may be taken to support the construction industry at this challenging time.
