1. Annual Data Return
This is the second year that Create Gloucestershire is asking its members to complete this Annual Data Return.
The aim is to gather basic data, across as many arts and cultural providers in Gloucestershire as possible.
We would like you to fill the form in using information from 2015/2016 financial year (April 2015 to March 2016).

This form can be saved, and you can return to it later as long as you do not have browser settings which delete your history. If you do, please either disable these settings, or collect your data using this paper version and submit it online in one go.
The section on volunteers, apprentices, interns and other work experience placements is optional. If you have people working with you in any of these ways, please tell us what you can about them. If there are questions you can’t answer, you can say ‘don’t know’ or not answer the question.
