1. Cancer Care Reviews: Practice level baseline survey for London

As we continue to improve the long term quality of life of people with cancer, we need to ensure everyone with cancer has access to a Cancer Care Review and that primary care professionals are receiving the appropriate information about their patients, at the right time, so that they can manage consequences of cancer and its treatment.

The National Cancer Programme has commissioned Frontline Consultants to create a national baseline of CCR activity to further explore barriers to delivering CCRs in the community.

The outputs of this survey will help to understand variation, target support and plan for continued improvement in the quality of CCRs.

Please note this is not a performance related survey.

This is an extremely important piece of work which will create a consistent baseline across the country.  CCGs are encouraged to support this work to ensure a high and accurate response rate to the survey. The baseline will be used nationally and locally, so it is vital that the picture is as robust and detailed as possible. This can only happen with your help.

Please complete the survey to the best of your ability by
09:00am Monday 16th OCTOBER 2017.
