Why do we need this information?

This form is divided into four parts: About You, Equal Opportunities, and GMCA Districts.
 
The About You (Part 1) section allows us to get to know you as an artist, so we can tailor our opportunities. The Equal Opportunities (Part 2) and GMCA Districts (Part 3) sections are required because the Royal Exchange Theatre and Cardboard Citizens receives money from a number of public funding bodies (for example, the Arts Council and Esmee Fairbairn Foundation), and we are asked to report back information on the demographics within the network. 

Please choose the "prefer not to answer" option for each question if you do not consent to sharing the information. None of the information collected on this form will be used in any decision-making process for Cardboard Camp Opportunities. All data collated will be treated in accordance with the Data Protection Act (1998).
