1. The 'State of Waltham Forest'

This survey is for parents and carers in Waltham Forest of children and young people aged 0-25 years with SEND (Special Educational Needs and Disabilities).
 Waltham Forest Parent Forum are gathering evidence from parents which will shape the work we are involved in in the coming year. We would like to know your experiences in a range of areas: how well the LA/Health identify children's special educational needs and/or disabilities, how well they meet those needs and how effectively they improve outcomes for those children and young people.  This survey will help us to reflect the views of a wide group of parents. Please answer the questions about how things are at the moment for your child or young person.
If you have more than one child with SEND (special educational needs and/or disabilities) then please complete a new survey for each child/young person. The survey should take no more than 15-20 minutes to complete and the information you give us is confidential and anonymous. We will not pass names on.
However if you would like to be entered into a prize draw for completing the survey please add your email address at the end of the survey
There will be one prize of a £100 voucher on offer for anyone who wishes to enter the draw. The winner will be chosen at random from those who at their names at the end of the survey after the closing date.
If a question is not relevant to your circumstances, please tick the' Not applicable box'. Please add additional comments as these are as important, if not more so than the individual answers.

* 27. FORUM DEVELOPMENT:
Based on parental feedback in 2020/21, there are several areas we feel we should concentrate on over the next year.
Which do you think are the most important? Tick all those you think should be priorities
We are already involved in work on Preparing for Adulthood, the Autism Strategy , the Therapies review and an ADHD strategy so we haven't included these areas in the list

Question Title

* 28. Part of the forum's on going strategy is to make contact with more families every year.
To do this we are hoping to introduce a 'Parent Champion' role where parents can talk to other parents either in their school or within their particular community( Mosque, Church, Synagogue etc) about the forum and to gather parental feedback about services- this may involve arranging coffee mornings in schools or taking groups of parents to coffee shops to gather feedback and then to feedback to the steering group.
If you feel this is something you would like to do please leave your name and contact details below and we will get back to you

Question Title

* 29. Do you have any suggestions as to how we can reach more communities?"

Question Title

* 30. Parents forum reps attended 688 hours of meetings over the course of 2020/21 and we expect that this year there will continue to be a high number of meetings where parental inclusion is needed. So that we have the capacity to cover all the meetings where we feel it is important to feedback parental concerns, we would like some additional parents to join our 'pool ' of reps to be used in a variety of meetings. It would involve attending meetings, workstreams , usually with at least one other parent on a topic of interest. The reps who attend would be expected to feedback a short paragraph of the content of the meeting and any actions for the forum. How many meetings you attend would be dependent on the project you are working on- most are time limited so it may be one meeting a month or a term.
If you would like to be a forum rep, please add your name and email address below and the areas you are interested in i.e education, health, social care, early years, mental health, CAMHS, short breaks etc
