Dear Registrant

As organisers of the UK & Ireland Prostate Brachytherapy Conference this year we would like to conduct a survey of current clinical practice in permanent and temporary prostate brachytherapy in the UK.

We hope this will provide a clear picture of the development of brachytherapy in the UK over the last few years and should allow comparison to the previous RCR audit from 2012†.

We would be grateful if each site could complete the questionnaire, with as much information as possible. There are drop down menus for most answer choices, but also the facility to add more information if needed.

If sufficient feedback is obtained the results will be summarised at the Belfast meeting and submitted for publication. The information may also be submitted to IPEM for inclusion in a Scope report in the Autumn.
