About this survey

We are working with the NHS to better understand children and young people's experiences of using crisis mental health services in Barnet, Enfield & Haringey.

A mental health crisis is when you feel your mental health is at breaking point. You may have had panic attacks or experienced extreme anxiety, been thinking about hurting yourself, or started having delusions or hallucinations for the first time.

Not everyone who experiences a mental health crisis has a diagnosed mental health condition (such as depression, bipolar or schizophrenia), however some young people do have a diagnosis, and may already be receiving treatment, support or medication.

We are interested in your experiences of using services that might help you in a mental health crisis, such as visiting the A&E department of your local hospital.

We know that it can be difficult to share your experiences of mental health crisis and that you might not want us to be able to identify you.

You can still take in the survey even if you don't want to give us your name or contact details - only give us as much information as you feel comfortable with! 

All of the information we get from the survey will help to shape the improvement of mental health crisis services in your local area.

The survey is aimed at 8 to 25 year olds who have experienced a mental health crisis in the last 5 years. We realise that younger children may need the support of parents or carers to complete the survey and would ask that in these situations the child leads the response.

The survey should only take you about 15-20 minutes to fill in.
