PARENT/GUARDIAN of all fencers who are competing at the event.

PLEASE NOTE WE WILL NEED ONE FORM PER CHILD IF YOU HAVE MULTIPLE CHILDREN COMPETING AT THE EVENT. YOU CAN COMPLETE THE SURVEY AS MANY TIMES AS NECESSARY FOR EACH OF YOUR CHILDREN BY FOLLOWING THE SAME LINK.

Please confirm you are the parent/guardian of a fencer competing at the 2017 British Youth Championships

* 5. Please list the name of the person at the venue responsible for your son/daughter throughout the BYC finals in Sheffield and their relationship to the fencer (e.g. parent, coach etc).

* 6. Please provide a contact number for the person listed above as being responsible for your son/daughter throughout the event.

* 7. Please provide an emergency contact name and number for an alternative person not in the venue for your son/daughter (i.e. different from above).

* 8. Please list any information about your son/daughter the organisers should be aware of (e.g. medical problems, injuries, allergies, hearing problems etc)

* 9. Having qualified for the BYC Finals I hereby consent to my son/daughter taking part in the British Youth Championships Finals on 29/30 April & 1 May 2017.  

I accept that neither the Organisers nor British Fencing can be held responsible for any accident, loss or damage
sustained at the Championships.

I understand that all persons wishing to take photographs or videos at these events must register their name, address and telephone number with the organiser at the event. I must also notify the organisers of the event if myself or my child wishes to withhold their permission to be photographed in close-up, but realise that it is impracticable to prevent general photographs from including my child.

I wil ensure that my son/daughter is familiar with the British Fencing code of conduct which can be found at
