Medical Registration for Archdiocese of St Andrews & Edinburgh Lourdes Pilgrimage - 7th - 14th July 2017

This form has been created to allow you to register with the Archdiocese of St Andrews and Edinburgh Lourdes Pilgrimage Doctors for our pilgrimage from 7th to 14th July 2017.

If you become unwell while in Lourdes, you must be registered with the medical team to receive advice and treatment. This form allows you to give us information, prior to travelling, regarding your contact details, current and past medical history, current medication and allergies. This will allow us to treat you safely while on pilgrimage. All hotel pilgrims, whether you have any previous medical history or not, will be required to complete a form to ensure safety.

Unfortunately, due to changes in legislation, if you do not register prior to travelling the medical team will NOT be able to offer advice or treatment if you become unwell. In the event of requiring medical treatment this would need to be arranged via a French GP or at the local District General Hospital.

If you have booked via Tangney Tours and completed the medical questions on the back of the booking form then you are already registered with the medical team and there is no need to complete this form.
