Action on Hearing Loss Cymru wants to know about your experiences of using adult social services in Wales.

 Adult social services usually offer information and advice and provide or fund social care and support such as:
  • assistive equipment for the home e.g. flashing doorbells or smoke alarms.
  • communication support or other specialist support to help you take part in the activities you enjoy and access local services.
  • help with daily tasks such as washing, cooking and cleaning.
Your feedback will help us develop new guidance for councils on providing high-quality adult social services for people who are deaf.

This survey is for people who are deaf (including people answering the questions on behalf of a family member or someone they care for). The survey asks a range of questions, including questions about your deafness.

The information you provide will be used for research purposes. Your answers are anonymous and you will not be named in any published survey results. By answering each question, you consent to us using this information as described above. You can choose which questions you answer and do not have to answer every question.
