Living Well Consortium have been awarded funding through BBC Children in Need to deliver Creative Well-being activities and sporting activities to Children and Young People aged 11 to 18 in the local community.  To register your interest for taking part in the activities please complete this form, one of our session leaders will then contact you to ensure you are given the information needed to access the session.  The information we are asking for is for monitoring purposes only.
