Understanding your health needs to make a difference

We want to ask a few questions about the past few months to understand your experience during lockdown and the impact of Covid-19 on your mental wellbeing.

We will use this information to inform our service support and share the key themes to inform commissioners about needs.

Your information is confidential and you can choose not to answer any questions

There are 39 questions, mostly multiple choice, the whole survey should not take more than 15 minutes to complete and we can support you to take part.

Thank you for your time and helping us to understand how to provide the best support for our communities.

Keep safe and well.

About the BAME Mental Health Collaborative:

Our Aim: To support systemic change in how mental wellbeing services are designed and delivered to people who experience inequalities and ensure better outcomes for people of BAME communities.

The Collaborative works as a sub-group of the Mental Health Provider Forum and accountable to Mental Health Partnership Board.

If you have any questions about the Collaborative, there is a contact form at the end of this questionnaire.
