APP Lancashire and Cumbria Peer Support Survey |
Introduction
We need your help! If you have used APP's peer support as part of the Lancashire and Cumbria project in partnership with Lancashire & South Cumbria NHS Foundation Trust, please fill in this survey.
It will take 5-10 minutes to complete.
Your time to complete it will help us improve the peer support that APP offers as part of the Lancashire and Cumbria project, and it will help us report to our funders on the importance of funding our work. We would like as many of our users to complete it as possible in order to demonstrate how important the APP Lancashire and Cumbria peer support project is, as it is a new project.
Some questions refer to the usefulness of the peer support service and benefits of it existing, and some we are required by our funders to ask for equality monitoring purposes. If there is anything you do not want to answer, please skip to the next question.

All surveys are anonymous and confidential though we do ask for your contact details at the end if you want to continue to receive peer support from ourselves.
We would like to use direct quotes taken from the surveys in reports and to raise awareness of APP's peer support service. All quotes will be anonymous.

APP abide by the General Data Protection Regulations (GDPR). If you have given your personal data, this will be stored securely, and is only available to the APP staff and APP volunteers coordinating groups or providing peer support. We will never pass your personal data onto anyone else without your written permission.
