How you can help
We are exploring how well the annual health check process is working for people with learning disabilities and if anything can be improved. If you are a parent, carer, or support worker of an adult with a learning disability who lives in Newcastle, please complete this survey and share your experiences.
We would be grateful if you would share the first part of your postcode (see monitoring questions at the end) so that we can identify any areas of good practice and any problem areas.
How we will use your information
The information we gather will be used to produce a public report on our findings. The report will be shared with the people who plan and provide our NHS services.
All information will be anonymous, treated confidentially, and held securely in line with the Data Protection Act 2018 and the General Data Protection Regulation (GDPR).
