This survey is all about understanding how easy it is to access places you want to go, from planning in advance to the experience you have while you are there.

We want to use the survey to highlight the issues people are facing and to see how we might develop our service moving forward.

We use the phrase 'places to go' to describe venues you can visit as a member of the public, so pretty much anywhere excluding workplaces and where you would need an appointment.

We would love to hear your experience of accessing places in your town or city and places you have visited.

We are really grateful for your time in completing the survey. If you would like to enter our prize draw please fill in your details at the start of the survey and tick the box, you can also sign up to our newsletter if you would like us to keep you updated on our work. We promise not to pass your details on.

Thank you!
