What can our Care Co-Ordinator do for you?

Our friendly Care Co-ordinator, Sarah Davey, works confidentially with individuals, families and carers to help improve the quality of life of our patients by signposting to a large number of social and voluntary agencies for help. She works closely with our GPs, community team, social services and hospitals to co-ordinate patient care and will keep you informed of any progress.
