For users of Taxis or Private Hire Cars with wheelchair or/and a helping dog.

* 1. Do you travel (or attempt to travel) in a taxi with

* 4. How often do you use a taxi or Private Hire Car (PHV) (roughly)?

* 5. In the last 12 months how many times has a taxi or PHC refused to take you because of your dog or wheelchair?

* 6. It is now illegal for a driver to refuse to take you unless they have a medical exemption certificate issued by the licensing authority i.e. the Council (e.g. for an allergy to dogs, or physical inability to handle a wheelchair). Was an exemption certificate?

* 7. If not, did you make a note of the driver’s details and report it to the Council?

* 8. If you did not report it, why not?
